I hereby acknowledge that I am voluntarily participating in the bungee fitness class provided by Ni Liu and Wenjing Zhang. I understand that bungee fitness Class involves certain inherent risks, including but not limited to, falling, tripping, collision with objects or other participants, and potential injury.
In consideration of being allowed to participate in the bungee fitness class, I agree to assume all risks associated with this activity, including those risks caused by the negligence of others. I acknowledge that it is my responsibility to consult with a physician prior to participating in this class to ensure that I am physically fit and able to engage in such activity.
I hereby release, waive, discharge, and covenant not to sue Bungee Fitness, its instructors, employees, agents, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the bungee fitness class.
I understand and agree that this waiver and release of liability is binding upon myself, my heirs, executors, administrators, and assigns, and that I voluntarily waive any right to bring legal action against Bungee Fitness for any injury or damages suffered as a result of my participation in the bungee fitness class.
I have read this waiver and release of liability, fully understand its terms, and voluntarily agree to its provisions.
